The AMSOIL dealer application form is very simple to complete but there are a couple of things to be aware of...

After you have the form downloaded you will notice three sections to the form. The first section you have to complete is your name, address, phone number, and email. After this information is complete you then proceed to the option #2 section.
Option #2 of the dealer application is the dealership option. This option requires a social security number and date of birth because AMSOIL will be mailing commission checks (or direct deposit) and at the end of the year AMSOIL will send you the earnings documents you need to complete your income tax filing.
When the dealer option is selected on the form you must sign and date the form then mail it into the AMSOIL address in the form.
After a few days you will receive your starter kit in the mail directly from AMSOIL headquarters. The kit you receive depends on which of the three options you select from the form.
